Special Health Conditions/Needs

Allergies: Food/Latex/Insect: Student with an allergy that requires the use of Epinephrine and/or antihistamine (Benadryl/Zyrtec).
 
  • A new Allergy Plan is required from their healthcare provider EVERY school year.
  • To attend any field trip, the following is required:
  • Current Allergy Action Plan signed by a healthcare provider on file with the school nurses every school year
  • A completed permission to carry and self administer Epinephrine form, signed by both the student and parent/guardian, must be on file with the Nurse’s Office, or you may indicate approval on your child’s emergency card for them to carry and self-administer Epinephrine.
  • It is highly recommended to keep extra epinephrine injector/spray (with pharmacy label) in the Nurse’s office

Asthma: Including Exercise Induced or Seasonal

  • A new Asthma Plan is recommended from their physician every school year
  • To attend any field trip, the following are required:
  • Your child must have and carry their prescribed inhaler
  • A completed permission to carry inhaler form, signed by both the student and parent/guardian, must be on file with the Nurse’s Office, or you may indicate approval on your child’s emergency card for them to carry and self-administer their inhaler.
  • It is recommended to keep an extra inhaler (with pharmacy label) in the Nurse’s office

Diabetes

Before the start of the school year, the following must be on file in the Nurse’s Office:

Required Documentation

 

  • Current Diabetes Care Plan from your child’s doctor (please request several weeks in advance to allow time for completion).
  • If your child independently manages their diabetes at school, please have the endocrinologist document this in the care plan.
  • Signed Parental/Guardian EMERGENCY Medication Form yearly on file with the school nurses
 

Required Supplies (as applicable)

 

  • Emergency medication (Baqsimi, Gvoke, or Glucagon – pharmacy labeled)
  • Continuous Glucose Monitor and extra supplies / glucose meter with test strips
  • Insulin pump supplies (site change supplies, tubing, cartridges/reservoirs, batteries, insulin vial, syringes)
  • Insulin pen or insulin vial (pharmacy labeled)
  • Fast-acting carbohydrate source (labeled snacks/juice)
  • Extra Pen needles or syringes
  • Lancet device and lancets
  • Ketone strips (and meter if blood ketone testing)
 

Please notify the Nurse’s Office of any changes to your child’s insulin or diabetes regimen.

Epilepsy

  • Please inform the Nurses' Office if your child has a Seizure Disorder.
  • A new Epilepsy Plan is REQUIRED from their physician EVERY school year
  • Emergency medication (with pharmacy label) must be kept in the Nurses' Office
  • Signed Parental/Guardian EMERGENCY Medication Form yearly on file with the school nurses
  • Encouraged to keep a few doses of your child's daily medication (with pharmacy label) in the Nurse’s Office
